Sponsor Form Template

The Sponsor Form Templates maintenance document is used to establish and maintain numeric codes and textual description values which are stored in database tables.  This data serves as reference information that is made available for display and selection in various areas of the system, including other e-docs.  The Sponsor Form Templates maintenance document allows for the creation and updating of templates for sponsor forms that are required for proposal submission.

Edit Sponsor Form Templates Section

The Edit Sponsor Form Templates section of the Sponsor Form Templates maintenance document is a unique section that allows you to specify and maintain the details associated with the reference table record in fields.

Description

Package Number

Required.  Type the suspected value in the box and click the direct inquiry icon or click the lookupsearchicon icon to search for and return the value to populate the box automatically.

Page Description

Required.  Enter the textual name of the page to identify it as it would commonly be referred to.

Page Number

Required.  Enter the numeral used to mark the relevant sequential page within the form template.

Sponsor Code

Required.  Type the suspected value in the box and click the direct inquiry icon or click the lookupsearchicon icon to search for and return the value to populate the box automatically.

Attachment

Click the Browse…button to look for, select, and upload the desired file.  The field displays the path and filename you selected and the attachment is uploaded upon save.
